1. Insurance Type
Select the relevant insurance category (e.g., health, dental).
2. Insurance Information
Provide officer’s name, phone, fax, and email for communication.

Select the relevant insurance category (e.g., health, dental).

- Company Name & Insurer Name: Enter provider and policyholder details.
- Member’s ID & Service Date: Add patient ID and service date.
- Claim Tracking Number: Unique identifier for the claim.
Provide officer’s name, phone, fax, and email for communication.
4. Service Information
- Service Name & Co-Pay: Add service provided and patient’s share.
- Provider & Codes: Enter provider info, CPT, and diagnosis codes.
- Billed & Paid Amounts: Specify charges and payments.
- Notes: Add remarks if needed.
